Civil and Family Summons Forms

The civil summons form (JD CV 1), revised in October, 2015, and the family summons form (JD FM 3), revised in February, 2015, now include a section for the plaintiff to provide an email address and to agree to accept electronic service of pleadings in accordance with Section 10-13 of the Practice Book. Older versions of the civil and family summons forms do not include this section.Plaintiffs are strongly encouraged to update the summons forms that they are using by going to the forms page on the Judicial Branch website ( and downloading the current version of the summons forms.

Section 8-1 of the Practice Book requires that a writ of summons be on a form substantially in compliance with the judicial branch forms prescribed by the chief court administrator, as such forms shall from time to time be amended.
